Service Coordinator

As a Service Coordinator, you will help individuals with disabilities and their families gain access to needed medical, social, educational, and other community resources to gain the greatest degree of independence and inclusion possible.

SUMMARY

  • Manages a caseload of varying ages, disabilities, and case complexity.
  • Identifying the needs of the person being supported and creating an action plan that will support the person in meeting those needs.
  • Matching the unique support needs of individuals and families (identified in the plan) with resources in the community.
  • Monitoring and assessing the quality, timeliness and effectiveness of services and support received through an ongoing review process.
  • Complete type written case note documentation for all activities completed for each individual within three business days of TCM activity, maintaining a minimum of 120 TCM billable hours per month.
  • Building relationships, and networking internally and externally.

 

EDUCATION & EXPERIENCE

  • A Bachelor’s degree from an accredited college or university or Registered Nurse license
  • Strong documentation, organizational, interpersonal, and computer skills required.
  • Experience in working with individuals with intellectual and/or developmental disabilities in a community-based setting as a Registered Nurse, or in social work, special education, psychology, counseling, vocational rehabilitation, physical therapy, occupational therapy, speech therapy, or a closely related area.
  • Case managers employed by a qualified provider on or before June 30, 2018, shall remain qualified.
